How to Document a Visit
Team Members can document client visits via the mobile application.
Screen Navigation
- Select Client: Click the drop-down arrow to select client
- Client Information ribbon
- Click the down arrow box to expand the client’s profile
- Click the up-arrow box to hide the client’s profile
- Month ribbon: Click the left and right arrows to select month and view visits by date of month
- Visit details
- Date of Visit
- Visited By
- Duration of Visit
- Time of Visit
- Start Visit button
My Past Visits
The user can see all client visits for a particular month by selecting ‘My Past Visits’ from the Select Client drop-down. My Past Visits only display visits documented by the logged in user.
Start Visit
This function allows user to record the details of the visit
- Start Visit button:
- Upon clicking Start Visit, the user is prompted to proceed, otherwise click ‘No’ if the Client is a No Show
- The timer begins upon clicking ‘Yes’
- The user can navigate to any other area of the VCP while the timer is running – including navigating to Medication Logistics
- Pause: The pause button allows the user to pause the timer; to resume the timer, press Resume
- Reset: The reset button allows the user to reset the timer back to zero (00:00)
- Type of Visit: Select the type of visit
- Visited Persons: Select the client and/or the collateral contact with whom the user visited
- Visited By: Select the team member(s) by whom the client was visited Add Notes: Enter optional memos for the visit
- Objectives Reviewed
- If the client has an Active Treatment Plan, select the objectives and goals discussed during the visit
- Billable Visit: Check this box if the visit is considered billable
- Interventions: Select intervention(s) from the drop-down menu
- End Visit: Click the End Visit button to complete the visit and end the timer
Visit Location
Upon clicking End Visit for an In Person visit, the user is prompted to either confirm or change the visit location
- If the location is accurate, click Confirm and the visit is documented at the listed location
- If the user needs to change the visit location, click Change
- The user has the option to manually change the address OR
- Select an existing address from the drop-down menu
- Only addresses entered in the Client’s profile display in this menu for selection
- Click save and then Confirm again
Current Visit
While documenting a visit, the user can navigate back to view that client’s prior visits. Click the Current Visit menu at the top of the screen to navigate back to the main visits page. Click the Open Current Visit button to return to the current visit.
Client No Show
- Answering ‘No’ to the proceed prompt allows the user to document when a client does not show for the visit
- The timer is removed, and duration is defaulted to 0 minutes
- The user is prompted to Select a Reason from the drop-down menu
- Click Save
Edit Visit
To edit a visit, click the edit icon next to the visit row OR click on the visit row and then the edit icon in the top right corner
All the details of the visit can be edited, including the location of the visit.
