How to Document a Shelter Visit
If the team is enrolled as a Shelter Team, the Visits drop down lists Shelter Visits as an optional menu. Shelter Visits are designed to enable team members to document visits back-to-back with minimal interruption.
- Select the Shelter Name from the drop-down menu
- All clients that have been assigned to that Shelter within their profile address are displayed for selection
- Select each client that will be visited at the Shelter
- When visiting with the client, press the start [play] button
- The timer begins and the visit location is captured via the mobile device’s geolocation services
- If the client has an upcoming Medicaid Recertification expiration date, the user will be prompted accordingly
- Press the pause button to stop the timer and start a new visit with a different client
- Tap the arrow to expand and select the client’s collaterals, visited by drop-down, and add notes
- Click the End Shelter Visits button to end all visits and complete entry of the visit details
- Any client that does not have a visit started will be documented as a No Show visit, prompting the user to select a No Show Reason > click Next
- The user can add notes for each selected client > click Next
- If a client has an active treatment plan, the user can select reviewed goals and objectives
- The user can select if the visit was a billable visit and any applicable interventions > click Save
- The user is prompted to confirm the visit location that was captured upon clicking start > click Save
