How to Run the Treatment Plan Reports
The purpose of the Treatment Plan Report is to view a list of all Active Treatment Plans by client. Each client’s assigned case manager – the team member who is responsible for maintaining the Treatment Plan – is displayed, along with the Effective Date and Next Review Date. The purpose of the Treatment Plans by Client Report is to provide detailed information about a specific client’s Treatment Plans, including any visits that took place during the Treatment Plan period and which goals and objectives were reviewed.
Treatment Plan Report
- Select Team: Select team(s) or select all
- Client’s Selection: Select client(s) or select all
- Next Review Date Range: Select the date range to query for the Treatment Plan Next Review Date
- Plan Details: Click the details icon to open that client’s treatment plan for more information
Treatment Plans by Client Report
- Select Team: Select the team
- Select Client: Select the client
- Select Effective Date(s): Select one or many Treatment Plan Effective Dates to view the information for the Treatment Plan(s)
- Treatment Plans are shown in order of Effective Date
- Each Treatment Plan can be expanded or collapsed using the arrow
- The Treatment Plan Goals and Objectives are displayed
- The Visits during the Treatment Plan are displayed, grouped by Team Member
- If more than one team member visited a client, view that information by clicking the team icon
