How to Run the Client Demographics and Health Details Report


The purpose of the Client Demographics and Health Details report is to query for specific data sets amongst all clients in the clinic. Click the Export Excel button to export the report results into a spreadsheet for further analysis.  



There are several parameters in this report: 

  • Select Team(s): Select team(s) or select all
  • Zip Code: Type in a zip code to search for clients with that zip code in any of their addresses
  • Age Range: Slide the bar or type in an age range to search for a specific age range

 

  • OR/AND Toggle: 
    1. If the OR toggle is turned on, the filters search for clients with any one of the values selected. For example, Medication 1 OR Medication 2 OR Medication 3
    2. If the toggle is turned off, the filters search for clients with all criteria selected. For example, Medication 1 AND Medication 2 AND Medication 3
  • Each of the below parameters are optional to the user. If more than one is used to search, the report will return clients that meet each of the parameters selected
    1. Select Diagnoses: Single or multi-select diagnoses to search for clients with a specific diagnosis. This parameter queries both Primary and Secondary Diagnosis fields
    2. Select Triggers: Single or multi-select triggers to search for clients with a specific trigger
    3. Select Medications: Single or multi-select medications to search for clients with a specific medication
  • Show Only Active Clients:
    1. If this toggle is turned on, only active clients are included in the report
    2. If the toggle is turned off, archived clients will be included in the report along with Discharge Date and Reason for Discharge
  • Show All Demographics:
    1. If this toggle is turned on, all demographic data columns are by default included in the report results
    2. If this toggle is turned off, certain demographic data columns are hidden from the report results
  • Select Columns: click this icon  in the report column header to select or deselect certain columns from the report results. Those columns in grey are considered core demographic data columns and cannot be hidden.

 

  • Sorting: the following columns can be sorted ascending or descending by clicking on the arrow next to the column name
    1. Client
    2. DOB
    3. Medicaid Recert Expiration Date
    4. Date of Admission
    5. Date of Discharge

The report groups clients by team and provides a count of the number of clients that meet the criteria.

The Primary Diagnosis and Secondary Diagnosis columns display the ICD-10 code. Click on the code to view the full description.

 

A client may have multiple medications and triggers. To view all medications or all triggers, click the underlined list within the report results.
